El 21 de noviembre de 1963, la Primera Dama y el Presidente se embarcaron en un viaje a Texas con varios objetivos en mente. Esta era la primera vez que se unía a su esposo en un viaje de este tipo en los Estados Unidos.  Después de un desayuno el 22 de noviembre, tomaron un vuelo muy corto en el Air Force One desde la Base de la Fuerza Aérea Carswell de Fort Worth hasta el Love Field de Dallas, acompañados por el gobernador de Texas John Connally y su esposa Nellie.  La Primera Dama llevaba un traje Chanel rosa brillante que había sido seleccionado personalmente por el presidente Kennedy.  Una caravana de 15, 3 km los llevaría al Trade Mart, donde el presidente tenía previsto hablar en un almuerzo. La Primera Dama estaba sentada a la izquierda de su esposo en la tercera fila de asientos en la limusina presidencial, con el gobernador y su esposa sentados frente a ellos. El vicepresidente Lyndon B. Johnson y su esposa lo seguían en otro automóvil en la caravana.

Después de que la caravana giró hacia la calle Elm en Dealey Plaza, la Primera Dama escuchó lo que pensó que era el ruido de una motocicleta y no se dio cuenta de que era un disparo hasta que escuchó al gobernador Connally gritar. En 8, 4 segundos, dos disparos más sonaron, y uno de los disparos golpeó a su esposo en la cabeza. Casi de inmediato, comenzó a subir a la parte trasera de la limusina. El agente del Servicio Secreto, Clint Hill, más tarde le dijo a la Comisión Warren que creía que ella había estado buscando a través del baúl un pedazo del cráneo de su esposo que había volado. Hill corrió hacia el auto y saltó sobre él, dirigiéndola de regreso a su asiento. Mientras Hill se paraba en el parachoques trasero, el fotógrafo de Associated Press, Ike Altgens, tomó una fotografía que apareció en las portadas de los periódicos de todo el mundo. Más tarde ella testificaría que vio fotos de ella subiendo por la parte de atrás. Pero no recordaba eso en absoluto.

James William "Ike" Altgens (/ˈɑːlt.ɡənz/, 1919 –1995) was an American photojournalist, photo editor, and field reporter for the Associated Press (AP) based in Dallas, Texas, who became known for his photographic work during the assassination of President John F. Kennedy.

Altgens was on assignment for the AP when he captured two historic images on November 22, 1963. The second, showing First Lady Jacqueline Kennedy toward the rear of the presidential limousine and Secret Service agent Clint Hill on its bumper, was reproduced on the front pages of newspapers around the world.

—When you look back at that image and you must see it so often I guess, what goes into your mind, what do you feel about the future of America and this family that you´d become so close to?

—That´s one of the photographs that really reminds me how sad the occasion was. That happened to be the day of the president´s funeral but it also was John John´s third birthday. And he had learned how to salute. When the president´s casket was brought out and placed on the …all the military present saluted at the president and then Mrs. Kennedy leaned over and whispered on John´s ear, said something. He threw his shoulders back and saluted his father. I don´t think there was a dried eye from the highest ranking general to the lowest private in the entire group that was there. And still when I see it, even today, I get choked when I see that photograph.

—It´s completely understandable, everybody choked and still does. Let´s talk about Jacqueline Kennedy. You stayed with her for another year. Did you ever talk to her again about this assassination or was it something that you didn´t  discuss?

—That was a subject that neither she nor I discussed with each other. We did not talk about the assassination at all.

—There´s a very powerful note that you have, a handwritten note from the First Lady, at the back of your book, which is full of the most extraordinary pictures and beautifully written and designed. And the note says:

For Clint Hill

Who did more than anyone to

make my life with the President happy

and who guarded and protected him until

the very end. How can I thank you.

Jacqueline Kennedy.

—That must have made you very proud after all the miseries of what you´ve gone before.

—Yes, it was very pleasing to me that she would take the time and write that note to me and indicated to me that I helped her in some way. Her life there during that 3 year period, in the White House when they were there, was the happiest period of her life.
